FITTING INSTRUCTIONS 'The Thumbs' Method: 1. Bring water to boiling point. Hold guard by strap; dip into water until guard is completely covered. Hold in water for 30 seconds. 2. Remove from hot water. IMMEDIATELY place in the mouth around the upper teeth. It will not burn. Using your thumbs, push guard up and back until rear molars are covered; then bite down firmly (not hard) to make custom impression on bottom of guard. Push tongue against the roof of the mouth; suck out air and water until a tight fit is obtained around the teeth. 3. Remove from mouth and place until cold water to set fit. 4. If you feel your mouthguard does not fit correctly, repeat steps above. 5. If necessary, use scissors to trim mouthguard for length, and trim corners for comfort. 6. Store in protective container. Use the enclosed name label to identify your SHOCK GUARD.

INSURANCE/WARRANTY INFORMATION: MOUTHGUARD MUST BE FITTED IN ACCORDANCE WITH MANUFACTURER'S PRINTED INSTRUCTIONS. BITE THROUGH AGREEMENT: Free replacement within one year of purchase. Return all defective mouthguards to Mueller Sports Medicine, Inc. DO NOT RETURN MOUTHGUARD TO DEALER. WARRANTY: The mouthguard warranty registration, along with a copy of the dated receipt of purchase, must be returned to Mueller Sports Medicine, Inc. within 10 days of purchase. The certificate of warranty is only valid for a period of one year after the date of purchase. No payment shall be made after one year. Treatment must be provided by a licensed dentist within 30 days after the date of injury. Payment is made only if expenditure for services provided by a licensed dentist exceeds any valid or collectable insurance. Payment is limited to $300.00 per injured tooth and total payment is limited to a maximum of $9600.00. Mueller Sports Medicine, Inc. must receive written verification of the injury within 30 days of the date of injury. The supervising coach or instructor must verify that the injury occurred in a supervised athletic contest or practice, and must verify that the user was properly wearing the mouthguard at the time of injury. CERTIFICATE OF INSURANCE DOES NOT PROVIDE WARRANTY FOR injuries resulting from (in competition or practice): professional athletics, hockey, sky diving/parasailing, field hockey, lacrosse, handball, racquetball, orthodontic treatment, preparation of a tooth to be used as an anchor for a bridge, injuries to the face, neck, jaw, skull or other body parts not attended to by a dentist; injuries to decayed unfilled teeth, or damage to dental jackets, crowns, bridges or bonding in the user's mouth at the time of injury.
