Welcome to ExtraCare Pharmacy & Health Rewards™! With this new ExtraCare benefit, you can earn up to $50 in Pharmacy & Health ExtraBucks Rewards™ annually for every member of your household who enrolls in the program. The purpose of this authorization is to allow CVS/pharmacy to record prescription earnings for each person; and because your privacy is of the utmost importance to us, this authorization explains how CVS/pharmacy uses and protects your information.

HIPAA Authorization

I hereby authorize CVS/pharmacy® and its affiliates to share my prescription and other health service records, including my email address, with the ExtraCare® program to enroll me in and administer the ExtraCare Pharmacy & Health Rewards™ program, and to inform me of new programs I may be interested in.

I understand that (1) my treatment, payment for treatment and eligibility for benefits does not depend on my signing this authorization; (2) after I sign this Authorization, some of my health information as described above, will become part of my ExtraCare Pharmacy & Health Rewards program record and thus no longer covered by the Federal Privacy Rule, but still protected under other consumer protection laws and CVS policy to not share such information outside CVS; (3) I have the right to cancel this authorization at any time by calling 1-800-SHOP CVS or at, but that my cancellation will not apply to any action that CVS has already taken based on this authorization before then; and (4) I am entitled to a copy of this authorization once signed by me.

Unless I cancel it before then, this authorization will expire one (1) year from today.

Frequently Asked Questions
– Do I need to join to earn rewards? I was already earning on prescription purchases.

The ExtraCare Pharmacy & Health Rewards™ program is the only way to earn rewards for your prescription purchases. Any prior prescription rewards program has been replaced by this program.

– Can I join online without a prescription number?

No. A prescription number, along with store number, date of birth and ExtraCare card number, is required so that CVS/pharmacy can properly match online users with ExtraCare membership to ensure that earnings and rewards are recorded for the correct person. If you do not have a prescription number, you will be able to join the program in any CVS/pharmacy store starting 2/1/13.

– How many prescriptions do I need to sign up?

Each person just needs to give us one prescription number to join. That'll help us link ALL of your CVS/pharmacy prescriptions to your ExtraCare Pharmacy & Health Rewards™ membership and you'll get credit when you refill them.

– Is there any reason why I can't participate in this program?

Everyone can join. Rewards cannot be earned for prescriptions purchased in Arkansas, New Jersey or New York; on any prescriptions for controlled substances purchased in Hawaii or Louisiana; or on any prescriptions transferred to a CVS/pharmacy® in Alabama, Mississippi or Oregon.

– Why do I need to sign a HIPAA Authorization?

The HIPAA Authorization allows CVS/pharmacy® to record the prescription earnings of each person who joins the ExtraCare Pharmacy & Health Rewards™ program.

– Can I sign the HIPAA Authorization for everyone who shares my ExtraCare® card?

A separate authorization is necessary for each person who joins ExtraCare Pharmacy & Health Rewards. Adults 18 years and older must sign individually; parents or guardians can sign for children under 18.

– Does each person sharing the ExtraCare card have to earn 10 credits before the $5 Pharmacy & Health ExtraBucks Rewards will print?

No. While you earn credits individually, the rewards will be paid out once the combined total of credits on the card reaches 10.

– How will I get the rewards I earn?

$5 Pharmacy & Health ExtraBucks Rewards will be issued approximately 1 week after every 10 credits have been earned. Rewards will print on the receipt or at the ExtraCare Coupon Center. They're also available on the ExtraCare Savings & Rewards tab. (You'll need to have or create a account and attach your ExtraCare card to it.)

– I no longer wish to participate in the program. How can I opt out?
