I think the NP should not always have to have a trainee when they are doing the patients exam. Sometimes it is fine but other times patient may not feel well and just wants to see NP. Not the NP fault but maybe that is part of their job as being experienced employee aka NP. But patient would feel better if you gave them the option to decline trainee observation. As several patients are seen in a day so statistically most patients will be willing to have trainee during the exam.
