What you should bring to your DOT physical

Before your DOT physical appointment, you'll need to complete the Health History section of the DOT Medical Examination Report. We ask that you bring that report to your appointment, as well as your driver's license and medical records.

A DOT physical includes the following exams:

  • Hearing
  • Urinalysis
  • Vision

What are the DOT physical requirements?

The DOT physical requirements include:

  • A minimum visual acuity of 20/40 in each eye, with or without corrective lenses
  • Having at least 70 degrees of peripheral vision in each eye along the horizontal meridian
  • An ability to distinguish colors
  • Being able to hear a "forced whisper" at a maximum distance of five feet, with or without a hearing aid
  • A passing urinalysis, which checks for kidney disease, diabetes and other issues
  • A passing physical exam, which checks the heart, lungs, abdomen, spine, limbs, brain and more
  • Blood pressure below 140/90

Does a DOT physical include a drug test?

No, a DOT physical doesn't generally include a drug test. However, it does include a urinalysis, which might detect drugs. Your employer may also require a separate drug test.

How much does a DOT physical cost?
